BUILDING ELEVATION INFORMATION (SURVEY REQUIRED)

The responsibility of providing a number of elevations based on the building type selected is that of the surveyor, engineer, or architect.  From these elevations gathered, the agent is required to determine the lowest floor for rating flood insurance.


The surveyor, engineer or architect should complete Section C, but if it is left blank, the agent should contact the responsible person who completed the form and the insurance company underwriter with any questions.


Elevations of machinery and equipment servicing the building (water heater, furnace, air conditioner, etc) must be provided, regardless of its location, whether inside or outside of the building, elevated on platform or not elevated.


Where the surveyor, engineer or architect cannot access the crawl space, an estimate on item C2 may be left blank and the estimated measurements entered on the comments section (Section D).


Elevations in Section E are based on feet, except in Puerto Rico, where it is metric and the agent must convert any readings to feet.


SURVEYOR, ENGINEER OR ARCHITECT CERTIFICATION

Section D is the certification of the surveyor, engineer or architect that the information provided by them is correct and their signature and identification number are required—in some states, a seal may be required.

Section E - The difference between the highest and lowest elevation grade are required.  For the designated Zones, the property owner or his representative may completed Sections A, B & E.

Address and other contact information about the property owner are requested in Section F.  The party completing Sections A, B, C and E must complete this section also.


COMMUNITY INFORMATION

The local official authorized to administer the community's floodplain management ordinance may transfer elevation information found on existing documentation—such as from an older elevation certification form or surveyor letterhead—to Section C, and then certify this information by completing Section G.  A statement advising FEMA of this transfer of information must be made in the comment section of the EC.  Section G may also be used to certify Item E4.

Government, Politics and Spirituality

Origins of Nationhood and Government

All life emanates from the single source (God, Spirit...). On taking physical form we become distant from that unity and adopt a distinct individuality. The purpose of physical incarnation is to experience that disconnection from source and those things that can only be experienced through interaction with other incarnate entities. As such the knowledge of our origin is hidden.

In the beginning, incarnate entities existed independently. However, the mechanics of evolution demonstrated the benefits of co-operation. Working together each member achieves more than it could in isolation. Thus groups, tribes, nations, societies... were formed. To be effective each group needs rules, and a means of enforcing those rules (laws). Hence the need for leadership (government). At essence laws encourage selflessness by curtailing selfish behavior.

Governments have enabled enormous human progress on the material plane by providing the infrastructures that promote enterprise and creativity, eg who would bother to make an effort if his neighbor could simply come along and snatch the fruits of his labor? Government-provided health, education and welfare services underline the sanctity of life, a very Spiritual ideal. Without the benefits of formal government humanity probably couldn't have reached its current heights.

But there is a negative side to government. Most notably the wars perpetrated in "national" interest. Such behavior indicates Spiritual immaturity. Unfortunately the technological progress that created a whole a host of weapons of mass destruction outstrips our Spiritual growth, hence the tragic devastation caused by warfare. More subtly, all governments curtail individual liberty and freedom of expression to some degree.

Many forms of government have existed and continue to exist, ranging from the jungle-like dictatorship where the strongest holds dominion over all else, through to democracy in which everyone has a say on who leads, can lobby the leadership, and even run for the job themselves. Though democracy appears, and probably is, the most agreeable form of government, consider that most democratic governments enjoy (sometimes substantially) less than 50% of the possible votes, and are not the choice of the majority. Furthermore, even their supporters probably don't support every single policy and detail.

Politics and Spirituality

Politics is the process of proposing governmental policies and arguing the merits of those policies over others. That wild differences of political opinion have always existed and continue to do so proves only that there is no single best way. Indeed there is not even common agreement as to the role of government, never mind how it should be achieved.

Elections are a barometer of the current state of society's Spiritual progress. If one happens to live in a democracy should one participate, and if so, how? While on earth our Spiritual duty is to engage with earthly affairs (else we wouldn't be here). As election time approaches we ought to familiarize ourselves with the available options. You might find one choice better for you personally, while another favors society as a whole. If so, it becomes a question of degree, eg can you sacrifice a little extra tax in return for vastly improved public health services? Remember, abstention is also a valid option, though ideally it should come from a considered decision rather than apathy. In the final reckoning, be guided by your inner instinct.

Politicians and Political Parties Agree On One Issue!

The 2010 midterm elections are only weeks away. At stake are all the seats in the House of Representatives and over thirty senate seats. Polling research determined the menu of the issues, but as always the primary goal of each candidate is election or reelection. This midterm really focuses on the side dishes of regional and local preference, since the federal government's iron chef competition won't occur until 2012.

TV's Iron Chef requires contestants to feature a secret ingredient as the primary source of their finished dishes. Just as in pre-scripted professional wrestling matches those competing are actually told that their featured ingredient will be one of three items. The show's producers apparently have made a judgment that the show is edgier by misrepresenting the amount of choices. Now our primary political parties are employing the same production tactics.

Pretending that government legislative conduct is limited only to poll-winning issues empowers politicians to continue business as usual. When television ratings for a particular show decline the result is modified content or cancellation by the network. As ratings of government legislative participants continue lower and lower, both parties spice up the action by infusing a minor ingredient into their political concoction. This year's spice of choice for Republicans is the Tea Party, a dicey element that does not have a consistency of desired change. The Tea Party is truly American goulash seeking a different dish. Some are from the camp wanting to feature smaller government. Others want to change our tax system. They are in agreement that change is needed.

The Democrats changed tactics because (thanks to the 2008 election) they were turned loose to cook up the final dream dish dream that progressive liberals have dreamed about for almost a century. Stunned that the American populace has turned up their noses at the ultimate perfect concoction, they truly believe that with more time those same voters will appreciate their work.

A suggestion for the political Iron Chef showdown in 2012 is to introduce a totally foreign base ingredient. It could be something removed from the prep kitchen in the 1960s by President Johnson, something that strikes terror to all political operatives. What is that ingredient?

Accountability is an oxymoron when married to the word federal government. Some states do have limited accountability; however, elected members of Congress who suggest using accountability as a base ingredient are almost extinct as the dinosaurs. In fact oftentimes they are referred to as dinosaurs.

To prep accountability for the 2012 contest is so simple even Einstein could do it. It is a five-step process, two of which will be revealed here. They are truly the stock needed for the accountability battle. The other three steps are just as critical but have no value without the first two.

First, one of the best pieces of legislation in this decade was a bill of less than one hundred pages - the Sarbanes Oxley Act. Corporate accountability was legislated almost organically. The Enron panic extinguished lobbying; pig squeals were heard far and wide across the land. The choir singing in unison was led by the chambers of commerce and other palace guard serving special-interest groups. It is significant to note that the Supreme Court ruled Sarbanes-Oxley constitutional, and not a single company (public or private) compelled to follow Sarbanes-Oxley's mandates has failed.

The black swans of the financial destruction were all flying with an exemption from SOX. Banks and financial institutions, primarily headquartered on Wall Street, were given an accountability bypass in the bill. The Federal Reserve Bank, an institution with more secrets than the CIA, certainly could not be entertaining any kind of accountable conduct mandates. Fannie and Freddie, those twin sons of different mothers, along with the insurance companies were also outside the law's coverage. In those short days of yesteryear we didn't realize they were outlaws because they presented themselves as our in-laws.

Amending Sarbanes-Oxley requires only one sentence: The federal government, the Federal Reserve Bank, insurance companies and all investment banking or banking institutions are required to comply with Sarbanes-Oxley. Consider how discomforting the squeal of the corrupt corporate pigs was. Magnify that a hundred times because that one sentence will turn up a chorus of hogs.

Step two concerns conduct required in Sarbanes-Oxley that must be restated to guarantee compliance and produce true accountability. It's also one sentence in length: All institutions that now fall under Sarbanes-Oxley as amended must use generally accepted accounting principles (GAAP) to compile mandated filings.

For over 15 years the Association of Certified Public Accountants has requested that GAAP be used for federal government accounting. They did not request this system for the Federal Reserve Bank because it is unknown what they account for.

Those two short sentences guarantee a 2012 election that will feature meaty issues for the first time in fifty years. If our new Congress will legislate these two modifications, along with three others to be revealed later, our grandchildren may have a future. Refusal guarantees the circus tent collapsing on our society.

Understanding Careers That Require Continuing Education

If you are in the process of making a career choice or thinking about a career change, there are a lot of things to consider. You need to find an option that will provide you with enough income to pay your bills and support your family. Ideally, you will choose a career that is fulfilling and brings you some happiness, even if you experience occasional boredom or burn out. Finally, you must find something you are willing to commit to, especially if you opt for something that requires continuing education. Some people work in environments where you have the occasional meeting or consultant came in to give you tips on performing better. But there are other jobs that require a day or more each year of continuing education. These fields require greater commitment because you must continue to learn and grow as your profession changes. If you are a parent and notice your child is off for an in-service day, this is probably your child's teachers get continuing education.

Similar to teaching is the childcare field. While childcare providers may not need as much continuing education as teachers, they are still required to turn in a certain number of hours or credits each year.

Again, the state regulations vary from one state to another but most require that childcare providers stay up to date with theories, lessons, safety issues and discipline techniques. Continuing education is also a great way to feel rejuvenated about your work and go back into the classroom or learning environment with a renewed attitude and spirit.

Sometimes, sales positions require continuing education. There are typically no government regulations, but your company may require you stay up to date with sales techniques and information about your product. Your employers will more than likely pay for this continued training because it will help you on the sales floor, ultimately making them more money.


Regardless of your profession and its requirements, continuing education is a good idea. Those who are self-employed should seek out opportunities to continue to grow so their business is more successful. The Political Class - Do As I Say, Not As I Do

One always get a little suspicious when a politician tells one you should do something (pay more taxes, use less gas, etc.) for yourself, your country, or the nebulous "common good." One reason for that suspicion is that many, many times the actions of those same politicians are inconsistent with their own actions.

This point was driven home recently in a news report from the Mexican newspaper, La Razon. The newspaper alleged that Hugo Chavez, the very, very socialistic semi-dictator of Venezuela had placed $137 million of his family's wealth in five bank accounts in the United States. His office, of course, denied the reports, claiming that Mr. Chavez is not a rich man with only one house to his name and no car after he gave his car to his son. I do not know if the news report is true or not but I doubt that Mr. Chavez is quite as poor as he claims. How ironic it would be that as his country's economy suffers and his people suffer, this socialistic ruler was reaping the rewards, allegedly.

That brought to mind the various examples of hypocrisy that our own politicians have tried to hoist upon us without hoisting their opinions and dictates upon themselves:

- Late last summer, Bill and Hillary Clinton threw a huge wedding party and reception for their only child, Chelsea Clinton. Most news reports estimated that the gala cost between $3 and $5 million. I would never tell anybody how to spend their money, especially as it relates to their kids. however, I do find million dollar weddings way over the top, especially when shortly before the extravaganza Secretary Of State Hillary Clinton opined in the press that some Americans make too much money.

If this is indeed the case, one could opine that anyone that can spend upwards of $5 million for a wedding makes too much money. The Clintons have made well over $100 million since he left office but still have the audacity to say some Americans make too much money. Obviously at $5 million for a wedding and over $100 million for their earnings, she hypocritically does not include herself in that category.

- On one of the television networks today there was a story on the office shuffle that is going on in Washington as new people come to serve in Congress and others leave. One aspect of the story was that Senator Minority leader Mitch O'Connell was going to move into the office space that had been used by the late Ted Kennedy. No big story here except for the fact that the office space for this one Senator was a whopping 3200 square feet in size. Most Americans have never lived in a home, never mind had an office, that was 3200 square feet. Most have never worked for an executive that had an office that was 3200 square feet.

As politicians talk about downsizing government and the need for the country to tighten its belt in these tough economic times, how about showing that same attitude and downsizing their own budgets and extravagances? No one in government needs an office that big. Break it up into smaller offices, bring in some cubicles like the rest of America and pocket the savings. Don't tell me to downsize when you have 3200 square feet to bounce around, whether the occupant is a Democrat or a Republican.

- A favorite piece of hypocrisy from the past year comes from the good Senator from Massachusetts, Senator John Kerry. Recall that Mr. Kerry has married into the Heinz family fortune, which puts his family wealth well north of $100 million. Also, Mr. Kerry lives in Massachusetts, summers in West Yarmouth, Massachusetts and has represented the state of Massachusetts in the Senate for years. Seems like he is Massachusetts citizen through and through.

However, according to a July 23, 2010 Boston Herald news article by Gayle Fee and Laura Raposa, that may not be the case. Apparently, the Senator recently purchased a luxury 76 foot, New Zealand built boat with "glossy varnished teak interior, two VIP main cabins and a pilothouse fitted with a wet bar and cold wine storage." Obviously, this is not a row boat. However, rather than berth in the boat in Massachusetts where he lives, where he summers, and who he represents in Congress, Senator Kerry decided to berth the luxury sloop in Newport, Rhode Island. This action allowed him to avoid paying almost $440,000 in Massachusetts sales tax and an annual Massachusetts excise of about $70,000. Rhode Island does not levy those types of taxes on boats.

So here we are, just a month or so removed from the Democrats howling that those making $250,000 or more in America do not pay enough taxes and he obviously is dodging paying legitimate, existing taxes to his financially strapped home state with this little out-of-state berthing maneuver. How hypocritical can you get? He is trying to avoid in state taxes what only a small handful of Americans make in a single year. And it is not like he needs the money, given the Heinz fortune he has access to. Do as I say, pay more taxes, while I look to avoid the same fate.

- Back to the Clintons, we mentioned above that they have made over $100 million over the past decade or so. Regardless of how much money they make, they continue to get full Secret Service protection at the taxpayers' expense. However, for many of those retired taxpayers collecting Social Security, earn a penny more than what the political class dictates in a year as a threshold of earnings and start paying back a dollar to the government for every two dollars over that maximum earning level. Thus, the common American can be penalized for earning too much money but our rich politicians are never required to step up and pay for their protection regardless of how rich the became as a result of their time in office.

- Staying with the Clintons, recent news reports and Congressional testimony by and about Hillary show a brazen disregard for taxpayer dollars and the taxpayers that pay them by the State Department. In Congressional testimony in February, 2010, it was revealed that the United States government was about to embark on building a $1 billion embassy in London. $1 billion for a single building. That was not to be confused with the $500 million embassy that the United States government was building in Afghanistan's capital.

Is the State Department and Congressional committees that allow this type of reckless spending to exist not aware of the hard times Americans are suffering and the skyrocketing national debt the political class is running up? $1 billion for one building is ridiculous as is 3200 square feet of space for a single politician. I guess these people are just special.

- How could we forget the early days of the Obama administration where it seemed that every day we found that nominees for high ranking administration positions never got around to paying their fair share of taxes? Ordinary Americans go to jail for not paying their taxes but somehow these political appointees got away without jail time. Tom Daschle, a long term ex-U.S. Senator never got appointed because of the large amount of taxes he owed but one nominee, Tim Geithner, actually became Secretary of Treasury.

- One last action, although minor, reflects on how out of touch the politicians in this country are with the needs and issues of the ordinary citizens. Given the turbulent November midterm election results and the low approval ratings of politicians in general, you would have thought they would have gotten he message: start working on the major issues and stop working on wasteful government spending and actions that benefit no one except the political class. Democrats in Washington took a "shellacking" (Obama's words).

But one New Mexico politician did not think he needed to heed these results. Governor Bill Richardson has spent his time and government resources of his state to work towards a government pardon of Billy The Kid over the past few months. Billy The Kid was an outlaw, there is no doubt about that, and he died almost 130 years ago, no doubt about that. But with public schools failing, unemployment stubbornly high, state governments running out of money, etc., Governor Richardson would rather work on pardoning a long dead outlaw. Get the message governor, you are not exempt from a low approval rating, work on the real issues.

One could go on and on, the hypocrisy of the political class knows no end. Unfortunately, their elitist attitudes and out-of-touch insular lives makes it very difficult for them to understand how real Americans are suffering and that maybe they should not tell us what to do while they lounge around in billion dollar embassies and 3200 square foot offices.

Several steps from might help to set them straight:

  • Step 1 - reduce the budget and size of the Federal government by 10% a year for five years. Maybe if the political class had less of our money to work with, embassies would be smaller, offices would be smaller, and the political class would have to learn, for the first time, to live on a budget like the rest of us do.
  • Step 2 - hold Congressional committee members accountable for government screw ups, removing those committee members from their posts for those screw ups. This would force them to finally do some work and truly understand the consequences of their actions: penalties or dismissal for non-performance, just like the rest of working America.
  • Step 3 - base Congressional pay raises on performance, as measured by the voters, so that the voters could punish hypocritical and excessive, elitist behavior by the political class in addition to rewarding straight forward ands effective governance by the political class.
  • Step 4- impose term limits on all politicians so that they reason for gaining a political seat is not to see how big your office could eventually become by staying in office an unlimited amount of time but to see how much you can contribute to the good of the country, while in office a limited amount of time.
  • Step 5 - require ex-Presidents to start paying back one dollar of every two dollars earned, above a minimum threshold, for their Secret Service protection, just like the rest of us have to do with our Social Security payments.

Hopefully, with these steps and a better class of politicians we can return to the original tenets of the Constitution and Bill of Rights, where every American is treated the same way and given the same opportunities, whether or not they are an elected politician of not.

We want you to understand and be satisfied with the terms of this Contract.  As you read through it, remember that the words "we", "us" and "our" refer to Your Insurance Company, Inc., the insurer (hereinafter referred to as YOUR INSURANCE COMPANY).  We use the words "you" and "your" to mean you, the Insured and your Covered Dependents.

To assure coverage exists from employer-provided plans, annual W-2 reporting beginning for the 2011 tax year requires disclosure of the value of each employee's health insurance provided by the employer.

Small businesses with fewer than 25 employees and average wages of less than ,000 are eligible for tax credits beginning in 2010. The credit is up to 50 percent of employer contributions for health insurance premiums. Employers with fewer than 10 employees and average wages of less than ,000 receive a 100 percent credit. Wages paid to five-percent owners of a business (or 2 percent of S corporation shareholders) are excluded from the average calculation.
